Appointments

If you are a new patient, you must fill out paperwork and fax, email, or deliver it into our office and provide us with your driver’s license and insurance information. Once we have your paperwork and verify your insurance will schedule your appointment.

If you are an existing patient, call us at (410) 766-1544 to set up an appointment.

All cancellations must be made 24 hours prior to the appointment. Failing to do so will result in a $25 missed appointment fee.
